An AHCA-licensed Health Care Clinic cannot simply continue indefinitely without the required director. AHCA says a director change must be reported within 21 calendar days from the effective date, and its change package requires documentation for both the incoming and outgoing director. A resignation starts a clock, not a grace period to ignore.

A transition date, a replacement physician, the agreement and attestation, a background result, the license, and the prior director’s resignation or termination letter — filed in coordination with the clinic’s AHCA consultant.
